Meet Our Team


No one person can provide for all the needs of hospice patients and their families.  Based on that assumption, hospice care is provided by a team of people with different specialties working together.  This provides a more comprehensive and responsive form of care benefiting both the patient and family.  The patient and family are the heart of the team, which also consists of the following people:


Attending Physician
Description - Patient's regular physician who is a key member of the hospice team.

Responsibilities - Assumes responsibility for medical management of treatment throughout patient's involvement in the hospice program. 


Hospice Medical Director/Clinical Director
Description - Works with the hospice staff and regular physician.
Responsibilities -Manages the hospice program at the local office level.  Works with hospice staff and regular physician to assure adequacy of trained staff and to assure adequate patient care and education. Makes routine home visits to assess patient.


Registered Nurse
Description - R.N. assists in the management of the hospice program at local office level.

Responsibilities - Trains/supervises caregivers, assures adequacy of patient care, assures adequate patient education, and assesses patient at least weekly.


Certified Nusing Assistant
Description - CNA Assists with hands-on patient care in accordance with the Plan of Care and assists in maintaining a safe and clean environment under the direction and supervision of a RN.


Social Worker
Description - Assists the patient and their family with social resources.
Responsibilities – Identifies resources, provides assistance with legal documentation or guidance, and provides support for the patient and family.   The social worker is trained in end-of-life issues and is available for every patient and family.  The social worker provides emotional support for anticipating grieving, assessing coping skills, assists with community resources, financial issues and funeral planning.


Chaplain
Description – Offers spiritual support of the patient, their family and clinical staff.

Responsibilities - Offers spiritual support for patients and their family.  Assures adequate bereavement and continuous counseling for families after the passing of the loved one.
